Payments & Fees
Engaging in psychotherapy is an investment of effort, finances, and time. I am an in-network provider with United Health Care and Cigna; please check with the insurance provider directly to determine if I am in-network with your specific plan. I can provide a SuperBill for you to submit independently to your insurance company if I am not in-network. This special receipt can help you receive direct reimbursement available, should your insurance plan provide out-of-network benefits. To see if your insurance plan offers out-of-network outpatient mental health benefits, call the member number on the back of your insurance card.
Also please note that to utilize insurance benefits, you, your child, or family member will have to meet the requirements for a mental health diagnosis. If you wish to be a private pay client, a diagnosis is not required for services.
Payment Policy
Fees For Services
Biopsychosocial Assessment $150 per 70-minute Session
Individual Therapy Sessions $135 per 50-minute Session
Family Therapy Sessions $200 per 90-minute Session
Method Of Payment
I require a written payment agreement and credit card on file. Payment is due at the time of service. Acceptable forms of payment are: exact cash amount, check (made out to Cardinal Counseling Services, LLC), or credit/debit card (MasterCard, Visa, American
Express, and Discover).
Return Check Policy
Insufficient-funds checks will be returned upon full payment of the original amount plus $25 for any returned check. If a check is returned for insufficient funds, I will require that you pay using cash or credit card only from that point forward.
Late Cancellation and No-Show Policy
Due to the demand for sessions and respect of time scheduled. You will be charged the full fee for cancelled sessions with less than 24 hour notice.
